 We will gladly file your claims to your insurance carrier. In order for the insurance company to allow us to submit the claims we must have the card holder and patients personal information. This includes but not limited to: Social Security number, birthday, address, employer information, and insurance group number/policy number. Without this important information we are not able to access the information we need to check on your coverage and benefits.

Please contact your insurance to verify our office is in your network or call our office with the name of your insurance carrier. We will do our best to provide you with general information and network status. 

We do not accept HMO/DHMO plans. These plans assign you to certain providers. Please contact your insurance company for more information. 

We offer a treatment plan estimate for all procedures. Please remember that is is ONLY an estimate from what we have gathered from the information from your insurance. Guarantee of payment is only determined when insurance has accepted the claim and determined the amount they will agree to pay. It is your responsibility to know the limitations of you insurance coverage. Our office is not responsible if your insurance denies your claim for any reason. We will gladly help you fight if you feel something is not right but it is ultimately your responsibility.
