Directions

We are located at 3439 Keith Street NW in Cleveland, Tennessee. In the South East Bank Plaza (Old Shoney's Plaza) beside Baskin Robbins and directly across the street from Cooke's Food Store.

 


What To Expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling.

Be sure to bring completed paperwork, insurance card, ID, and a list of your medications or even bring the bottles themselves.  If you have had a joint replacement, please check with your surgeon to see if an antibotic is needed before all of your dental appointments.  If you are pregnant or think you may be pregnant, please call your OB/GYN and ask them to fax our office a clearnace letter for dental treatment. 

Also, take some time to review our staff page and familiarize yourself with the doctor. We look forward to your first visit.

We usually find that after you have completed your first appointment with us and you get home and look over your treatment you often have more questions.
